Birthmarks are common and come in a variety of shapes, sizes, and colors. Most people carry one or more – and the majority require no intervention. When treatment is desired for cosmetic reasons, the right approach depends on what type of birthmark it is. Vascular birthmarks and pigmented birthmarks are fundamentally different biologically, and they respond to different lasers.
Board-certified Manhattan dermatologist Dr. Brian Hibler offers individualized laser treatment for two of the most commonly treated birthmarks – port wine stains and nevus of Ota – with wavelength selection, energy settings, and treatment intervals tailored to the specific lesion and to the patient’s skin type.
Birthmark treatment is highly lesion-specific. Beyond port wine stains and nevus of Ota, other birthmarks are evaluated on an individual basis and matched to the appropriate approach.
- Café-au-lait macules – flat brown patches present at birth or in early childhood. Some respond to picosecond laser, though recurrence is common.
- Salmon patches (stork bites, angel kisses) – most fade on their own within the first years of life; persistent lesions may respond to vascular laser.
- Infantile hemangiomas – most involute spontaneously over the first years of life; active or complicated hemangiomas require pediatric dermatology referral, and residual vascular staining after involution may respond to VBeam.
- Congenital melanocytic nevi – evaluation for change over time is more important than cosmetic treatment, and larger or atypical lesions may require surgical excision or ongoing dermoscopic monitoring.
- Becker’s nevus and other hyperpigmented patches – treatment options are individualized and results vary substantially. Becker’s nevi may respond better when laser hair removal is first performed, followed by treatment of the pigmented lesion.
Most importantly, any birthmark that is changing in size, color, or texture, or that has become symptomatic, warrants dermatologic evaluation before any cosmetic treatment is considered.
A birthmark consultation begins with a full examination of the lesion, a review of the patient’s medical history and family history where relevant, and standardized photography. Dr. Hibler explains which wavelength or combination of wavelengths is appropriate, the expected number of sessions, realistic expectations for improvement, downtime, and cost.

Can all birthmarks be removed with laser?
No. Some birthmarks respond well to laser treatment, some respond partially, some do not respond at all, and some are better addressed surgically or monitored rather than treated. Vascular birthmarks such as port wine stains typically respond to pulsed dye laser, and dermal pigmented birthmarks such as nevus of Ota typically respond to picosecond laser. Other pigmented birthmarks (e.g. café au lait patches) have a more variable response to laser treatment.
What is the best laser for a port wine stain?
The 595nm VBeam pulsed dye laser is the established standard of care for port wine stains. Its wavelength is preferentially absorbed by the abnormal capillaries that make up a port wine stain, producing selective clearance of target vessels. Thicker, nodular components may benefit from treatment with a long-pulsed Alexandrite (755nm) laser. A typical treatment course is a series of sessions spaced one to two months apart, and lifetime maintenance may be needed.
What is the best laser for nevus of Ota?
The picosecond laser is the current treatment of choice for nevus of Ota. It delivers ultra-short pulses that shatter dermal pigment into fragments small enough to be cleared by the body’s own immune cells. In melanin-rich skin, conservative energies and longer intervals between sessions reduce the risk of post-inflammatory hyperpigmentation or residual hypopigmentation.
How many treatments are needed to remove a birthmark?
This varies substantially by lesion type. Port wine stains typically require six to twelve or more VBeam sessions, and lifetime maintenance may be needed. Nevus of Ota typically requires five to ten picosecond sessions. An individualized plan is developed at consultation.
Is birthmark removal safe in dark skin?
Yes, when appropriate wavelengths, energies, and treatment intervals are used. Melanin-rich skin is more susceptible to post-inflammatory hyperpigmentation with any laser treatment, and Dr. Hibler uses conservative energies, longer intervals between sessions, and physician-directed topical protocols to reduce this risk. Provider experience treating skin of color is a critical part of safe treatment.
Does insurance cover birthmark removal?
In most cases, birthmark removal is considered cosmetic and is not covered by insurance.
