Laser Tattoo Removal
While tattoos are designed to be permanent, many individuals eventually seek removal for a variety of personal, professional, or aesthetic reasons. Tattoo removal has become increasingly advanced and effective, allowing patients to significantly fade or eliminate unwanted tattoos with minimal damage to the surrounding skin. At his Manhattan-based practice, Dr. Brian Hibler, a board-certified and fellowship-trained medical and cosmetic dermatologist, provides expert evaluation and treatment plans for patients interested in tattoo removal.
The decision to remove a tattoo is deeply personal. Regardless of the reason, modern dermatology offers several tools and techniques to safely and effectively address these concerns.
Laser technology is the gold standard for tattoo removal. It works by emitting focused light energy that targets the pigment in the tattoo ink. This energy breaks the ink particles into tiny fragments, which are then gradually cleared by the body’s immune system over the following weeks.
Dr. Hibler uses advanced laser systems with multiple wavelengths to effectively treat tattoos of various colors and depths. Q-switched lasers and picosecond lasers are among the most common and effective tools for tattoo removal:
- Q-switched lasers: These devices deliver high-intensity pulses of energy in nanoseconds. They are effective for black, blue, and red ink and have been widely used for years in dermatologic practices.
- Picosecond lasers: Newer than Q-switched systems, picosecond lasers emit energy in trillionths of a second, providing more efficient breakdown of ink particles. These lasers are especially beneficial for stubborn ink colors and may require fewer sessions than traditional lasers.
For certain tattoos, a fractional ablative laser can be used. This laser drills microscopic holes into the skin, allowing for the ink particles to seep out of the surface of the skin. It can be combined with Q-switched or picosecond lasers for an enhanced effect.
Each session typically lasts between 15 to 30 minutes, depending on the size and complexity of the tattoo. Numbing cream or local anesthesia may be applied to minimize discomfort along with force chilled air for additional anesthesia.
Tattoo removal is not a one-time procedure. Most patients require a series of treatments spaced several weeks apart to allow the skin to heal and the immune system to clear the ink. Typically, results are seen over the following 4-8 weeks after each treatment; however, the tattoo may continue to fade over the following months without further intervention.
To improve the efficacy of treatment, a perfluorodecalin (PFD) patch (Describe®, Merz) may be used. When placed over the tattoo, the patch dissolves microbubbles that form on the surface improving light penetration. This allows for multiple passes to be made over the tattoo in a single session, providing enhanced clearance with each visit. The patch also protects the top layers of skin, making the treatment safer and expediting the healing process.
Tattoo removal is highly effective when performed with the right technology, by a trained medical professional, and with realistic expectations. While not all tattoos can be erased entirely, many patients achieve dramatic improvement in the visibility of unwanted ink.
