Under Eye Filler

The under-eye area is one of the most delicate and technically demanding regions of the face to treat. A small volume of well-placed filler can meaningfully refresh a tired, hollow appearance, and poorly placed filler can create lasting problems. Tear trough treatment is a region where experience and patient selection matters. Dr. Brian Hibler treats the under-eye region with a conservative, individualized approach using hyaluronic acid fillers chosen for the anatomy of the lower lid, and combines treatment with lasers and energy-based devices for the most natural rejuvenation.

Under-eye filler is the placement of a soft hyaluronic acid gel into the tear trough, the anatomic groove that runs from the inner corner of the eye along the junction between the lower lid and the upper cheek. When this groove deepens, light casts a shadow across it, which many patients perceive as a dark circle or a persistently tired appearance. Filler placed carefully beneath the skin restores support in this hollow and softens the transition from lid to cheek.

Hyaluronic acid is a sugar molecule that occurs naturally in the skin. As a filler, it provides gentle volume, attracts water for a hydrated look, and can be dissolved with hyaluronidase if adjustment is ever needed. That reversibility is one of the reasons hyaluronic acid is the preferred filler category for the under-eye region. The other is the need for a soft product that integrates nicely with the delicate tissue for a natural appearance.

Under-eye concerns rarely have a single cause. Common contributors include an inherited tear trough that deepens with age, loss of midface volume, thinning of the delicate lid skin that reveals underlying vessels, pigmentation within the skin itself, and prominence of the underlying fat pads. Filler is an elegant solution for hollowing and shadowing, but it is not the right answer for pigmentary dark circles, prominent fat pads, or significant skin laxity. The consultation will determine which factors are contributing to your appearance and recommend the approach most likely to deliver a natural result.

The best candidates for under-eye filler have a defined tear trough hollow, reasonably good skin quality of the lower lid, and no significant prominence of the underlying fat pads. Patients with prominent fat pads, significant lower-lid laxity, or a strong pigmentary component to their dark circles are often better served by a combined plan or a different treatment entirely. We will offer an honest assessment during the consultation, including declining treatment when it is not likely to serve the patient well.

Dr. Hibler uses only hyaluronic acid fillers in this area, and selects among several options based on skin thickness, depth of the hollow, and individual anatomy. Products commonly used in the tear trough include Restylane-L, Restylane Eyelight, Belotero Balance, and Juvederm Volbella. For very thin skin, an especially soft product reduces the risk of visible product or a bluish tint known as the Tyndall effect. For deeper hollows or patients who also need midface support, a firmer product may be used higher on the cheek to indirectly lift the tear trough.

For some patients who prefer a more natural approach, platelet rich plasma (PRP) injections may be performed to the under-eye. This is an option for very mild under-eye concerns without significant volume loss, though results typically require a series of treatments and need more regular ongoing maintenance.

Often, the most refined result comes from addressing the midface and the tear trough together rather than injecting the trough in isolation. Many patients notice correction when treating the lateral cheek, which can be a preferred approach to start here before injecting into the tear trough directly.

A tear trough treatment session typically takes fifteen to thirty minutes. The hyaluronic acid products used in this area contain lidocaine, which reduces discomfort. Dr. Hibler uses a blunt-tipped cannula rather than a needle for injection into the under-eye area. The cannula reduces the risk of bruising and vascular injury and allows the filler to be distributed evenly along the appropriate anatomic plane.

Mild swelling, tenderness, or a small amount of bruising is common and typically resolves within a few days. Most patients return to work and normal activity the same day. Final results settle in over one to two weeks.

Duration varies by product, individual metabolism, and the depth at which the filler is placed. In this area, most patients see results lasting between nine and eighteen months, and some see product persist even longer because the under-eye region is comparatively low-movement. Because hyaluronic acid fillers can be dissolved with hyaluronidase, any concern about the result can be addressed if needed.

The under-eye region carries a higher technical risk profile than most other areas of the face. Common side effects are mild and temporary and include swelling, bruising, tenderness, and small lumps that typically self-resolve or respond to gentle massage. Less common but important considerations include prolonged swelling, the Tyndall effect, visible or palpable product, and, very rarely, vascular complications including effects on vision. Dr. Hibler uses techniques designed to reduce these risks, including cannula-based injection, deep placement along the periosteum where appropriate, conservative product volumes, and staged treatment when the goal is best achieved gradually.

The most natural under-eye result often comes from combining treatments rather than filler alone. A comprehensive plan may include midface filler to support the lower lid from below, neuromodulators to soften crow’s feet, energy-based treatments such as lasers to reduce pigment and vasculature or to tighten skin, and a considered skincare regimen.
